First Time Patients, you will receive a link via email and/or text to create your JointStrong® Patient Profile prior to your first appointment. Your Jointstrong® Account will be used to fill out all necessary forms, review your plan of care, and communicate with your clinician between appointment times.

Auto-Related Injury Patients, please also make sure to complete the Patient Auto Form in your JointSrong® Profile. You will also need to bring your auto insurance card, claim information, and the date of the accident.

Please arrive at least 20 minutes prior to your appointment so you will have time to fill out these forms if you have not done so already.

Please also bring your photo ID and health insurance card(s) to all your appointment.
